Healthy Aging Roadmap Available

The UNC Institute on Aging is pleased to announce the availability of the The North Carolina
Roadmap for Healthy Aging: A Health Promotion Programming Guide for Providers. The document can be downloaded from the Roadmap web site at http://www.aging.unc.edu/roadmap/.

The Roadmap is the first step to organizing and coordinating efforts at the community level to
provide health promotion programming to older adults. It is designed for community and health-care providers interested in implementing health promotion programs to best meet the needs of older adults in their community. The Roadmap provides a step-by- step guide to the process of identifying a health promotion need in your community, identifying appropriate health promotion programs to meet this need, establishing partnerships to pool resources, and implementing programs.

This document is designed to be accessed and utilized electronically. Viewers will have an opportunity to provide updated programming information, updated county information, and feedback. The online version of the Roadmap will be updated, and will also be available through the Division of Aging and Adult Services website in the future.

The Roadmap is the result of a collaborative effort between the UNC Institute on Aging, the North Carolina Division of Aging and Adult Services, and the North Carolina Division of Public Health. The project was supported by the State-based Examples of Network Innovation, Opportunity, and
Replication (SENIOR) Grant, RFA #HA2006-II offered through the National Association of Chronic
Disease Directors.
